Attached is the pre order form. Please complete the necessary fields and submit it to epic@studentrate.com. You will get a confirmation when the order is received and reviewed. We are automating the process so there maybe changes to the order submission, however, all orders will be reviewed and submitted for processing. Cheers Edit, please see the updated order form with the upgrade option.

Alex, thanks for posting this and getting back to everyone with our questions but I have another question. I looked at the form and it looks like everything being asked on the form is for anyone who is signing up for a new account with Sprint. Should I still fill this out if I already have a plan with Sprint and I am just using my upgrade to get the phone? FWIW, I have the Everything Data Family Share plan with 1500 minutes and I am the primary line on the account.
Thanks again for everything!

Thanks again for everything! very sorry about that -- if you are upgrading please enter sprint username and password in the first section and skip the portion for the credit check and so forth.

Questions:
1c) Accessories - huh?
2Aa) PIN - huh?
2Ab) Can I use my primary email address or should I use my .edu email address?
2Ae) SSN - required? I'm not big on emailing my SSN, but would be happy to provide it to Sprint over the phone.
2B) I can get the new phone and service and test it out to ensure I get good reception at my house before porting my number over?

If I'm ordering a new family plan and I'd like to buy 2 Epics for that plan, how would you like me to indicate that on the form?
Thanks for all of your help Alex!

Sorry to be annoying, but where exactly should I put that? Can I just send you my Sprint username and password in the body of the email that I need to send to epic@studentrate.com or do I put it on that form somewhere. Is there anything else I should include or is that all? EDIT: Also, Adobe is telling me it can only save a blank copy of the PDF and it will not save any text I type into document.

Alex, thanks for all the work. I just downloaded the form and got several questions:
1. Is the Total Equipment Protection or Service & Protection required for this phone?
2. Should I indicate the Invisible Shield at the Accessories part?
3. What is a PIN for the account setting?
4. I'm an international student who hasn't got an SSN number, then what should I do about this part? Is there any additional information or documents needed?
Thanks.

Accessories - if you are ordering anything like a Bluetooth headset AND if you would like the car charger OR the shield. PIN - is what you would like your PIN to be so that you can access your account online when it is setup. SSN -- required for the check -- one of our team members will call to get that from you. 2B - absolutely you have the 30 day money back guarantee from Sprint.

Thanks. Hi the last part of your statement (4 international student). Please indicate in the SSN part and one of our team members will contact you on Monday to setup an account for you to deposit funds to, sprint will not allow you to have a post paid contract without this. 1 - TER or S&P. Total equipment protection is if you loose your phone break it drop it in the urinal or the bathroom so anything that is not covered by warranty this will cover it. S&P -- if your phone breaks beyond the warranty period you will get a replacement phone 2 - please let us know whether you want the shield or the car charger. 3 - so that you can access your account online please specify a PIN number

Please post all your questions here and it will be answered by one of us. Thanks Alex. I'm curious about how the order process will work with this pre-order. When will you get our credit card information since it's not on the form? Are you basically taking these orders and manually ordering the phones on the morning that they become available? Could you give me a little info about how this works on your end? I'm just trying to figure out if there is a benefit of pre-ordering vs going to Sprint Online the morning of release and using the StudentRate discount. Thanks again!

We are not grabbing the credit card information for security reasons. We are having Sprint Reserve the phones for us (so there is a benefit) not just obtaining the information so that we can enter the information the morning of (good question though). We have put down what Sprint wanted from our members that were interested in reserving the phone. Hope this helps.

Yeah, that helps. I'm going to be on vacation in Cancun (WOOT!) that week, but I will have phone and internet access. Just trying to figure out what kind of business I'll need to handle on launch day, if any. So, we pre-order and then sometime between now and the 31st we'll get more information from you guys about how we're going to pay Sprint for the reserved phones?

I just want to make sure I understand this all correctly: By filling out the preorder, I've essentially skipped the step where I'll have to buy it from the Sprint website? Or have you just put one on hold and I'll still have to go to the Sprint website later? If this does replace that step, then once I have the phone and the Sprint account, that's when I follow the rest of the steps from the deal page that say this: Send an e-mail to sprint@studentrate.com with your name, new Sprint number and your unique promo code you entered while making your Epic purchase. In addition all customers that are new to Sprint or an already existing Sprint customer that is adding a new line with the purchase of an Epic 4G will receive an Invisible Shield phone protector from Studentrate.Do I understand this all right? Basically, now that I sent you guys the preorder form, unless something unforeseen comes up, I don't have to worry about anything until I have my new phone?

I plan to move to sprint and choose a family plan Everything Data 1500 with 3 lines (only one epic). When i filled the form do i need to choose the 3 phones (where?), do iget the same treatment as with spront online, waived fee, save50 code. to port the numbers from my carrier can i do it after i get the phone or do i have to port them from the start (extra charge for porting after account is created?) also will i be shipped all the phones the same day or do i get the ones that are not pre-order first and then get the epic?
Forgot to ask Do i need to write the same pin in part 2a and 3? And do i have to put my info or my fathers since he is the one that is going to pay the account.
